    When you send in your “transfer of ownership” application and fee, you will have to include with it:

    • A “consent to transfer” form signed by the current owner
    • Four copies of your floor plans, signed and dated by you, the applicant
    • Articles of Incorporation, if any
    • A signed lease or proof of ownership of the premises where the license will be used
    • Property tax clearance from your county treasure

    If you plan to move the license to a new location, you will also need to complete a “transfer of location” application and send it along with the other items listed above.
